Care Planning

Obtain the knowledge and skills to create, record, review, and improve person‑centred care plans while understanding their purpose, legal and ethical requirements, consistent care delivery, collaboration, and advance care planning.

Learning Outcomes

• Learners will be able to write clear, personalised, outcome-focused care plans that are 
  tailored to meet the unique needs of each person including their culture, health and 
  preferences
• Complete stages of care planning from assessment and goal setting to reviews and 
  updates
• Document positive experiences, progress, and meaningful moments
• Accurately record and update care plans so they reflect any changes to a person's 
  needs
• Understand how record keeping links to safety, quality, and CQC inspection findings
• How to involve people and families effectively and capture their input accurately
• Learners will be able to work confidently and consistently to produce high quality 
  records that reflect real care
• Have an awareness of Advance Care Planning

• Individuals residing in Staffordshire
• Individuals living outside of Staffordshire funded/commissioned by Staffordshire County Council

A small basic lunch will be provided on the day, you can purchase drinks from the café on site.
