This e-learning course gives care staff an overview of the knowledge required to identify and respond to early signs of deterioration in residents, with a focus on frailty and soft signs. Learners will explore what frailty is, why it matters, and how to assess it using the Rockwood Clinical Frailty Scale. The course introduces the nationally recognised PIER framework - Prevention, Identification, Escalation, and Response - and provides practical guidance on using tools like Stop and Watch and SBARD (Situation, Background, Assessment, Recommendation and Decision) to support timely, effective communication and intervention.

Objectives
- Define frailty and explain its impact on health outcomes and care needs in older adults.
- Identify the purpose and use of the Rockwood Clinical Frailty Scale in assessing and monitoring peoples' frailty levels.
- Describe what deterioration is and why early recognition is critical in care settings.
- Explain the PIER framework (Prevention, Identification, Escalation, Response) and how it supports effective deterioration management.
- Recognise soft signs of deterioration, including physical, behavioural, and mental changes, and understand their significance.
- Use soft signs recognition tools, with a focus on the Stop and Watch tool, to document and monitor early changes in people’s conditions.
- Apply the SBARD communication framework to escalate concerns clearly and effectively to healthcare professionals.
- Demonstrate appropriate escalation actions, including when and how to involve senior staff or external healthcare services.
