This training introduces the SWAN model of care to guide compassionate and dignified end-of-life and bereavement care. It explores how the model supports patients and their loved ones through personalised actions, communication, and memory-making. Learners will gain practical insights into implementing the SWAN model across care settings, ensuring high-quality, individualised support at every stage.
