Learners will gain practical insights into implementing the SWAN model across care settings, ensuring high-quality, individualised support at every stage.

This training is suitable for all care support worker staff.
This training is equivalent to 30 minutes participatory hours for CPD. A certificate will be provided upon completion of this training.
This training does discuss death and dying, and may be upsetting for some audiences.
This training was developed by the MPFT Palliative and End of Life Care Team (Palliative + EOLC).
Objectives
- Discuss the background of the SWAN model of end of life and bereavement care.
- Understand what the aims of the SWAN model of end of life and bereavement care are.
- Recognise when the SWAN model of end of life and bereavement care could be used and how to facilitate this.
- Identify how the SWAN model of end of life and bereavement care principles can be adapted in care homes.
