Description
This webinar will identify the current national and local drivers guiding quality end-of-life care. It will enable people to recall the physical signs that help identify when end of life is approaching. Participants will be able to summarise the key principles of holistic assessment at end of life and discuss the importance of sensitive communication, including the involvement of patients and their families in decision-making. The session will highlight how individualised care planning improves patient experience and quality of care, focusing on the last days and hours of life, the physical signs of a dying person, and the importance of assessing the total person to develop an individualised care plan. The training outcomes of this course are aligned with NICE and Department of Health guidance.
