It provides a brief overview of the online information platform designed for adult social care services in Staffordshire and Stoke-on-Trent. The course covers the platform's features, including access to a wide range of supportive information, elements of the NHS directory of services (DoS), and links to various community teams. It also highlights the collaboration between the Staffordshire and Stoke-on-Trent Integrated Care Board, Staffordshire County Council, and Stoke-on-Trent City Council, as well as the support from local teams and organisations.  Read more

This Lite-bite course has been developed to introduce the topic of Person Centred Care Planning. It provides an overview of the essential principles and practices involved in creating care plans that are tailored to the unique needs and preferences of individuals. The course emphasises the importance of collaboration, communication, and respect in the care planning process. Participants will learn how to gather and document personal information, understand health and medical history, assess mobility and physical abilities, and consider cognitive and mental health needs. The course also covers topics such as nutritional needs, personal care preferences, social and recreational preferences, and spiritual and religious beliefs. By the end of this course, participants will have a foundational understanding of person-centred care planning and be equipped with the knowledge to implement these practices in their care services. It emphasises the importance of honouring individual preferences, whether it involves a specific hairstyle, choice of clothing, or wearing makeup, to boost self-confidence and sense of identity. The course also highlights the significance of promoting dignity and self-respect, which are crucial for mental and emotional health. Participants will explore practical ways to offer this support respectfully, sensitively, and tailored to each person's unique needs and preferences. Read more

This webinar will identify the current national and local drivers guiding quality end-of-life care. It will enable people to recall the physical signs that help identify when end of life is approaching. Participants will be able to summarise the key principles of holistic assessment at end of life and discuss the importance of sensitive communication, including the involvement of patients and their families in decision-making. The session will highlight how individualised care planning improves patient experience and quality of care, focusing on the last days and hours of life, the physical signs of a dying person, and the importance of assessing the total person to develop an individualised care plan. The training outcomes of this course are aligned with NICE and Department of Health guidance. The session is suitable for registered staff within care homes.
